Four questions that all read as “are you a nurse?”
Hospital applications ask about your license several times over, in wordings that look interchangeable and are not.

Do you hold an RN license?
The plain one. It asks about holding, nothing else.

Do you hold a multistate or compact license?
A different fact about the same license. Holding a single-state license is not an answer to this, and the two are not interchangeable on a travel posting.

Is your license unencumbered and in good standing?
This is about standing, not about holding. Somebody who holds a license can still have to answer this one carefully, and it is where disciplinary history is being asked about without the word being used.

Are you licensed in [state]?
A question about a jurisdiction. Being licensed somewhere is not being licensed there.
What this tool does with it
It answers the ones it can prove and abstains from the rest. A license you entered yourself, named the way the form names it, is an answer. A question about standing, or about a state, or about a compact when you recorded a single-state license, is left blank and flagged for you.
The uncomfortable version of that promise: it will park questions you could have answered in a second. Handing you a blank box is the failure mode we chose, because the other one files a claim about your license under your name.
